Request Your MMI Transcript

As an MMI alumnus, you may request your MMI Preparatory School transcript at any point in time. To request an official MMI Preparatory High School transcript, please click the button below, fully complete the Transcript Request Form, and submit it to Shannon Williams, Director of Human Resources and Registrar, via email, Fax, or mail. Transcripts will be processed usually within three business days.

Transcripts are only considered official if they are in a sealed envelope with a school registrar’s signature and date across the seal. Only the alumnus may request an official transcript, not a parent, spouse, or sibling.

Transcripts may be picked up at the MMI Main Office, or mailed directly to you or the school to which you are applying.

Complete the form in its entirety. Transcripts will be processed usually within three business days after payment is received. There is no fee for the first official transcript.

Request Your Replacement MMI Diploma

In case of loss, theft, or destruction of an original MMI Preparatory School diploma, replacement diplomas can be obtained by filing a request. To request a replacement send a letter stating the reason for the replacement and a check or money order in the amount of $50 (US) made payable to MMI Preparatory School.

Damaged diplomas MUST be returned in order to process the request.

  1. If a diploma is destroyed in fire or any other catastrophic event, we require a copy of the insurance report, police report or a notarized statement.
  2. If a diploma was stolen, we require a copy of the insurance report, police report or a notarized statement.
  3. If the diploma is lost in a move, we require a copy of the insurance report or a notarized statement.

Legal Name Changes: The original diploma must be returned or the guidelines above must be followed if the Diploma is destroyed or lost. A graduate cannot hold two diplomas. Requestor must also supply a copy of the court document or notarized letter confirming the legal name change.

Letters of request must include the following information:

  1. Full Name and your signature
  2. Diploma Name (name recorded on the original diploma)
  3. Date the degree was granted
  4. Shipping address including a telephone number

When the diploma is ready, it will be sent via mail to the requested address.
